I remember doing this in Delphi 7, but I don't remember how, or it is different in the new Delphi IDE. But how do I add a new template to the items gallery?
So then it will show up under the File / New menu.

Package projectSelect File > New > Package - Delphi. Select File > Save Project As and navigate to the folder location. Create a new folder, navigate to it, and save the project as DialogButtonsPackage. Right-click the newly created project group in the Project Manager and select Save Project Group As.
