I did not change my app icon assets whatsoever from my previous build(s), and have submitted at least 10 builds before without this issue. Also, per another SO answer, I submitted this app for review and it has been several hours and the icon still will not appear.
Is this an Apple problem, or am I doing something wrong in Xcode/iTunes Connect?
*Note my app has already been reviewed and approved 3 times by Apple, but the icon has always shown up in Connect
Edit: I was able to produce this strange message when I right clicked and opened the "image" in a new tab:

Drag the front, middle, and back icons to the “App Icon - App Store” set in the App Icon & Top Shelf Image folder. For the iOS target, drag an icon to the “App Store iOS” well. For the WatchKit App target, drag an icon to the “Apple Watch App Store” well.
Open Settings and under Manage app, search for the app whose icon is missing, and tap to open it. Do you notice an option to Start/Enable the app? It could be under the App Info menu, depending on the make and model of your phone. If yes, most probably, the app is disabled, and you need to re-enable it.
Press and hold the app icon until a menu appears and tap on Add to Home Screen. If you don't see the option to Add to Home Screen, it likely means the app is buried in a Home Screen folder or on a hidden Home page. In this case, you can press and drag the app icon back onto your Home Screen.
If you think you accidentally deleted the App Store, don't worry. The App Store cannot be deleted, so it's just a matter of finding out why the App Store icon is missing. One of the common causes for a missing App Store icon is that it's inside a folder on your phone or hidden in the App Library.
This is a problem / bug inside iTunesConnect. It will show up eventually, no need to worry. (I know that from experience.)
