I have 2 machines. On one of them I updated to xcode 4.5, but doing so uninstalled my iOS simulator for 5.1.
I'm hesitant to update my other machine as I have a project that requires the 5.1 simulator.
Are my assumptions correct or did I do something to remove the older simulator without knowing?

It's very simple. Open this folder in Finder, either by pressing CMD + SHIFT + G and copy-pasting this path /Library/Developer/CoreSimulator/Profiles/Runtimes/ or manually navigating to the above path. Once opened, you will see all the runtimes downloaded. Just delete them and we are good to go.
Now, go to /Library/Developer/CoreSimulator/Profiles/Runtimes and delete the files named iOS 9.3. simruntime and iOS 10.3. simruntime .
Open Xcode and click Menu > Xcode > Preferences > Select Components, and then choose the simulator version you want to download. When a simulator is opened from AppStudio, AppStudio Player automatically installs (if necessary) and opens in it.
You can redownload the simulator in the Xcode preferences.

Download Xcode 4.3.3 for lion and open the dmg, and click and show package contents,and copy the Contents\Developer\platforms\iPhoneOS.platform\sdks\iPhoneOs5.1sdk folders into your xcode4.5 Contents\Developer\platorms\iPhoneOS.platform\sdks folder, and it works
