I'm running the latest version of TFS 2015 Update 2.
I'm trying to create a test plan as per this guide https://msdn.microsoft.com/en-us/library/dd380763.aspx
Which suggests when I go to Test > Test plan there should be a green plus icon to create test plans and so on. However I don't see this...
I've got all 'test' related permissions under Security & Areas Allowed for my user, yet this makes no difference.
Am I blind or missing something obvious?

If you do not see a button to create a New Test Plan then that means 1 of 2 things: 1. Your user does not have an appropriate subscription to MSDN, Visual Studio Enterprise, or Visual Studio Test Professional OR 2. You do not have a Basic + Test Plans license (Go to Organization Settings > Billing to purchase).
Manage test plans and test suites under an area pathOpen the Security page for area paths and choose the user or group you want to grant permissions. Set the permissions for Manage test plans and Manage test suites to Allow.
A TFS Test Plan is a container for a testing effort. When you create one, you'll be asked to choose an Area Path and an Iteration. Area Path is an idea that's scattered around lots of places in TFS that allows you to keep things relevant to specific parts of your Team Project.
You need to Change access levels for your account to Advanced. If your account doesn't belong to Advanced access level, you won't be able to see the green plus icon.
